NP to work with the Oncology Services department acting as an advocate and educator for patients from point of entry, through dia! gnostic studies, diagnosis and treatment plan for cancer through survivorship.You will coordinate a patient\\\'s plan of care, including appointments, transportation, and education, serving as a liaison throughout the facility and in the community regarding services for this unique population. Clinical decisions are formulated in consultation and collaboration with attending physicians. In addition, you assume responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each cancer patient including support for interdisciplinary and cross facility collaborations e.g.
tumor boards and referrer communication. You are responsible for tracking, monitoring, and measuring all aspects of the patient navigation program. You are also highly involved in regulatory preparedness, and in leading accreditation programs specific to specialty.As the Nurse Navigator you will develop relationships with radiologists, surgeons, onco! logists, radiation therapists, hospitalists, nurses and other ! members of the interdisciplinary team to care for patients with a diagnosis of cancer.Ideal candidate has a Master\\\'s Degree â€" Nurse Practitioner or Clinical Nurse Specialist (CNS) with a minimum of 2 years of oncology nursing experience. Licensed as an APRN in the State of Texas and has a current RN licensure in the State of Texas.
Oncology Certified Nurse (OCN) certification should be obtained within one year of hirePosition reports to the Director of Oncology Services
