Health Texas Provider Network, formed in 1994, has grown to become one of the most effective physician-directed patient-centered organizations in the nation. Approximately sixteen hundred people are employed in our organization, including more than 500 physicians practicing in 156 locations. Patient visits in FY10 surpassed 1.3 million, including more than 95,000 new patients. Our practice is comprised of 57 primary care centers, 69 specialty care centers including 9 physiatric medicine centers and 3 pulmonology critical care centers, 7 hospitalists programs, 23 specialty satellite clinics including 12 liver disease outreach centers, 4 senior health centers, and a family practice residency program.
The Medical Records Coder codes ancillary and procedural outpatient, emergency room, outpatient clinic, physical medicine, interventional radiology records and other invasive outpatient medical records. Enters diagnostic and procedure cod! es into the coding/abstracting system and billing system as ap! propriate. Codes inpatient and day surgery charts if necessary as directed by management. Utilizes International Classification of Disease (ICD-9-CM) and Physicians Current Procedural Terminology (CPT) coding systems and other coding references to ensure accurate coding.
I. Analyzes and interprets medical records to ascertain complete and accurate information for coding.
II. Determines principle diagnosis(es) and procedure(s) utilizing updated Medicare coding guidelines and other references.
III. Identifies and codes all pertinent comorbid and complicating diagnoses utilizing the encoder.
IV. Identifies and codes all applicable invasive procedures utilizing the encoder.
V. Abstracts required data into the coding and abstracting system.
VI. Enters diagnosis and procedure codes into the billing system on series accounts to generate a bill, as appropriate.
VII. Serves as a resource person for the Health Information! Management Department as well as other Hospital departments by answering questions related to coding issues.
VIII. Works collaboratively with revenue cycle departments to ensure billing, code and edits are processed timely.
IX. Works closely with direct Manager to ensure operational needs of facility are met.
X. Utilizes the chart management system as required by task and responsibility.
Qualifications
High school diploma required; some college preferred
Certified Coder
3+ years of relevant work experience in a clinical / physician office
Vision and Centricity experience is highly preferred
Must have excellent customer service skills
Attention to detail
