Description
The individual in this position is responsible for screening all patients for medical nece! ssity of admission to the facility, regardless of portal of entry, once physician order for admission is received. If the physicians order for admission status does not match the InterQual review results, the admission nurse will call the physician for more information to assist in the admission process. The admission nurse will notify the physician of the results of status determination and either clarify the admission status or send the case for review to the PA for secondary review. The admission Case Manager will be responsible for reviewing and clarifying all admission orders for accuracy and completeness. The Admission Case Manager will review the surgery and procedure schedules the afternoon prior to the surgery and compare to the Medicare Inpatient Only List to obtain the correct physician admission orders. The ACM will be knowledgeable of community resources to refer patients that do not meet InterQual admission criteria when the physician agrees that admission is ! not appropriate.
The individuals responsibilities wil! l include, but not be limited to the following activities: a) obtaining valid physician order prior to bed placement b) medical necessity screening c) care coordination d) making the appropriate referrals to community resources e ) referring complex social issues to Social Service f) communicating with patients and their families g) collaborating with physicians, office staff and ancillary departments (e.g., lab, pharmacy) j) participating in weekly Complex Case Review clear, complete and concise documentation in eCCM m ) maintaining accuracy of patient demographic and insurance information h) identifying and reporting over and under utilization
Qualifications
Current licensure by the Texas State Board of Nursing
- Current BLS for health care providers
